Public health is not only about treating illness—it’s about creating an environment where communities thrive. For Jemez Public Health, this means a proactive, community-centered approach focused on prevention, empowerment, and cultural sensitivity.
At the heart of Jemez Public Health is the understanding that health extends beyond medical care—it’s about supporting the community in living healthy, fulfilling lives. Addressing Social Determinants of Health (SDOH) is key. SDOH are non-medical factors that influence health, such as access to healthcare, education, nutritious food, and safe housing. Jemez Public Health works to ensure every community member has the opportunity for a healthy life.
The Community Health Workers at Jemez Public Health are vital to this effort. These professionals provide in-home care and education, overcoming barriers like transportation and mobility issues. In 2024, the Diabetes Liaison program had a significant impact, with three liaisons conducting 583 home visits focusing on diabetes education and management. Additionally, our CHWs conducted over 1,000 blood pressure and blood sugar screenings, directly encouraging early detection and proactive health management.
One standout initiative is the Colorectal Cancer Awareness Program. By educating the community on the importance of early screenings, this program promotes awareness and provides tools like at-home screening kits. Furthermore, the Health Education Program designs culturally relevant campaigns to promote healthy behaviors. In 2024, the program published 29 blogs on topics like food sovereignty, generating over 2,200 views and reaching a growing audience. It also engaged youth through initiatives like the Hemish Youth Summit, focused on suicide prevention and food sovereignty.
When it comes to food sovereignty, Jemez Public Health ensures access to healthy food options. The Food Sovereignty Program revitalizes traditional farming practices, connecting people with food sources. In 2024, over 2,500 pounds of produce were harvested and distributed to the community, and 15% of the produce was served in school settings. Furthermore, a new beekeeping initiative was launched to support biodiversity and pollination.
Jemez Public Health is about more than just healthcare—it’s about creating a sustainable, informed, and resilient community. Through programs that educate, empower, and connect people to cultural roots, Jemez Public Health is building a healthier future for generations to come.
