Who We Are
The Jemez Public Health Nursing Program systemically organizes and delivers culturally sensitive health services to the Hemish community. The program consists of a multidisciplinary team that promotes healthy lifestyles through health education via one-on-one consultations, home visits, and community engagement. The team includes certified community health workers, medical assistants, a fitness specialist, diabetes liaisons, and a registered dietitian. They respond to priority health issues spanning from infectious diseases to chronic illnesses. Overall, the Jemez Public Health Nursing Program is concerned with protecting the health of the entire Hemish community while attending to their cultural and traditional needs.

Grocery Tours
Grocery tours provides community members with invaluable knowledge on how to read food labels effectively and navigate local grocery stores. By empowering individuals to make informed, budget-friendly food choices, Grocery Tours directly support glycemic control and healthy weight management efforts. Participants gain a deeper understanding of which foods align with their dietary needs, helping them make healthier selections that positively impact blood sugar levels. Hosted monthly.

Prevent falls and stay active with our fall-prevention services!

Designed to enhance safety and mobility for seniors, our program offers tailored exercises, educational resources, and home assessments to reduce the risk of falls.
With a focus on strength, balance, and coordination, we empower older adults to maintain independence and confidence in their daily activities.

Group Lifestyle Balance™
Discover a healthier you with the Group Lifestyle Balance™ Program! Our empowering community environment and expert-led sessions equip you with the knowledge and skills needed to manage your weight and lower your risk of type 2 diabetes. With a focus on mood regulation, portion control, physical activity, and nutrition, our program supports your journey towards glycemic control. Track your progress with pre and post-weight assessments, celebrating every success along the way. This program is generally hosted twice a year and lasts between 8 – 12 weeks.

Women's Wellness
The Women's Wellness Group offers a holistic approach to diabetes management and prevention, with a particular focus on supporting glycemic control.
This weekly, 8-week program provides participants a unique blend of integrative care and self-empowerment strategies to address mood regulation and behavioral health challenges associated with diabetes.
During sessions, participants are taught how to leverage art therapy techniques to regulate mood and address behavioral health concerns related to diabetes prevention and management. This creative approach provides a therapeutic outlet and equips individuals with valuable tools to manage stress, anxiety, and other emotional factors that can impact glycemic control.

Colorectal Cancer Can Be Prevented!

What is colorectal cancer?
​
The colon and rectum are part of your digestive system. Their function is to remove water and get rid of waste from the body.
Most colorectal cancers begin as non-cancerous growths called polyps that form on the inside layer of the colon or rectum.​Colorectal cancer often starts quietly without warning.
​
Colorectal Screening Tests
​
All men and women between the ages of 45-75 should have one of these two tests:

Screenings are done through the Fecal Occult Blood Test (FOBT).
You do this test ar home and send a stool sample to a doctor's office or lab every year.
All FOBT kits can be returned to Jemez Comprehensive Health Center. You may also call the
Jemez Public Health Office at (575) 834-7207 to have a Community Health Representative
return the Kit for you. All returned kits will be eligible for an incentive.

2. Colonoscopy
A test that allows a doctor to examine the inside of your colon and rectum for polyps or other
changes, at least every ten years.
​

Factors that increase Colorectal Cancer Risk
Most colorectal cancers occur in men and women AGE 45 or older.
​
PERSONAL HISTORY of colorectal polyps, colorectal cancer, or inflammatory bowel disease.
​
FAMILY HISTORY of colorectal cancer (mother, father, sister, brother or child).
​
